Gather All Required Documents

Before starting your claim, ensure you have the following:

Proof of Deposit (any of the following):

  • Passbook
  • Cheque book
  • Statement of account
  • Fixed/term deposit certificate

Valid Identification:

  • Driver's Licence
  • International Passport
  • National ID or NIN
  • Voter's Card
  • OR endorsed letter from a traditional ruler or Local Government Chairman

Mandatory for All Applicants

  • Recent Passport Photograph (clear and recent)
  • Active Bank Account Details (for payment)
  • Completed Checklist Form (required for manual submission)

Ensure all documents clearly show your full name and are legible.

Prepare Additional Documents (If Applicable)

Depending on your category, you may need extra documents:

  • Companies: Certificate of Incorporation, company seal, authorisation letter, IDs and passport photos of authorised officers.
  • Government Agencies: Authorisation letter, IDs, and passport photos of authorised officers.
  • Partnerships / Firms: Partnership agreement, IDs and passport photos of authorised partners.
  • Sole Proprietors: Certificate of Registration.
  • Associations / Clubs: List of trustees, resolution, IDs and passport photos of authorised signatories.
  • Deceased Depositor's Claims: Letters of Administration, Will or Court Order, death certificate, IDs and passport photos of beneficiaries.

Complete the Claim Form

Fill the Depositors Verification Form (CRD VF1) online or download it. You will be required to provide:

  • Your personal or business details
  • Name of the closed bank
  • Account type and deposit details
  • Contact information (address, phone, email)
  • Payment (receiving bank) details
  • Declaration confirming all information is accurate

All details must match your original bank records.
